The Gardens at Osage Terrace Assisted Living
The Gardens at Osage Terrace is the only Assisted Living II facility in
Northwest Arkansas.
The Assisted Living II facility is a joint partnership between the
Community Development Corporation of Bentonville / Bella Vista, who is the
building owner and Mercy Health System who is the service provider.
Assisted Living II is a residential setting that combines apartment living
with supportive personal and healthcare services allowing older persons
to maintain their independence and dignity while receiving a high level
of care and support.
The Gardens at Osage Terrace is regulated through the Office of Long Term Care,
staffed by a state licensed administrator, a registered nurse, L.P.N.s, as well
as multiple other disciplines to ensure the health, safety and independence of
the residents who reside there. The facility is a state Medicaid provider,
making living at the Gardens affordable for many low income seniors.
Medical staff are available 24 hours a day to the frail elderly as an
alternative to nursing home placement. The facility is also made affordable
through State Medicaid funding.

Basic Core Services
- 24-hour supervision by awake staff - 16 hours licensed nursing -
Registered Nurse Oversight
- Ability to provide or assist in obtaining emergency care 24-hours a day
- Assistance with social, recreational, and other activities
- Linen and housekeeping services
- 3 balanced, nutritious meals daily
Health Care Services
- Health care services identified as needed, such as regular monitoring of vital signs
- Medication administration
- Monitoring of independent medication regimes
- Arrangements for pharmacy services
- Periodic nursing evaluations

- One bedroom apartments are slightly higher

Financial Assistance
State Medicaid eligible residents utilize their monthly income to make
financial contribution to their room, board, and services. The state wavier
